CONSENT FORM
Photography, Video and Audio Recording

- ORIT UK Acquisitions Limited (“ORIT”)
(together referred to as “the Project Partners”)
This creative initiative is delivered within Scottish schools but managed by UK‑based organisations in compliance with UK‑wide legislation.
1. Purpose of this consent form
As part of the creative project, photographs, video footage, audio recordings and/or transcripts (“Recordings”) will be taken during workshops, activities or events.
The Recordings are required to document the project and to create end‑of‑project content, which may be used for promotional, reporting and communications purposes by the Project Partners.
This consent form applies to both pupils and adults (including school staff) who may be filmed, photographed or interviewed as part of the project.
2. Giving consent
I give permission for Recordings featuring myself or the child/young person named below (if under 16) to be recorded as part of this project.
I understand that this consent applies to Bonanza Creative, ORIT , and any professional creatives authorised to work on their behalf on this project.
Participation and inclusion
Participation in the project is not dependent on consent being given for Recordings.
Children and young people without consent will still take part fully in all project activities and will not be featured in any identifiable recordings.
3. How the recordings may be used
I agree to the Recordings being used for:
- Internal Use: Evaluation, learning, training and internal reporting within the Project Partners.
- External Use: Public reports, case studies, corporate websites and communications with stakeholders. Social Media: Digital channels including Facebook, LinkedIn and X (formerly Twitter).
The Project Partners will not knowingly use the Recordings in any way that is misleading, inappropriate or harmful.
The names of children or young people will not be used alongside images, video or audio recordings.
4. Copyright, ownership and duration
Copyright in the Recordings will be retained by the Project Partners for a maximum period of five (5) years from the date of capture after which they will be deleted or irreversibly anonymized.
Personal data contained within the Recordings will be processed, stored and managed in accordance with UK data protection legislation.
5. Children and young people (under 16)
Where the individual featured is under 16 years of age, this form must be completed by a parent, carer, or legal guardian with responsibility for the child.
By submitting this form, you confirm that you have the legal authority to give consent on behalf of the child named below.
6. Withdrawing consent
Consent may be withdrawn at any time by contacting Bonanza Creative.
Please note:
- Withdrawal of consent does not affect the lawfulness of any use made prior to withdrawal.
- Once withdrawal is received, the Project Partners will cease using the Recordings in future publications wherever reasonably practicable. Previously published materials may not be able to be retrospectively removed.

7. Data protection and privacy (UK GDPR)
Photographs, video and audio recordings are treated as personal data under UK GDPR. Bonanza Creative and ORIT act as joint data controllers for this project.
- The legal basis for processing this data is consent.
- Data is stored securely on password‑protected systems or approved cloud services.
- Access is restricted to authorised personnel and approved contractors working on the project.
- Data will not be sold or licensed to third parties outside the Project Partners.
You have the right to request access to your data, request corrections, or withdraw consent at any time.

This form may be completed and signed electronically.
By typing your name and submitting this form, you confirm that:

- Your typed name constitutes a legal signature
9. Governing law
This consent form is governed by the laws of England and Wales.
This does not affect the statutory rights of participants in Scotland under UK GDPR or other applicable UK legislation.
